Dixon Exit Criteria
Criteria Number Exit Criteria Description
Exit Criteria #1:
 
Demonstrated implementation and use of functional consumer satisfaction methods.
Exit Criteria #2: 
 
Demonstrated use of consumer functioning review method(s) as part of the DMH quality improvement system for community services.
 
Demonstrated planning for and delivery of effective and sufficient consumer services for adults (the Adult Community Services Reviews).
 
Demonstrated planning for and delivery of effective and sufficient consumer services for children and youth (the Children and Youth Community Services Reviews).
Exit Criteria #5:
 
Demonstrated provision of service to children and youth (aged 0 – 17 years).
Exit Criteria #6:
 
Demonstrated provision of service to children and youth (aged 0 – 17 years) with serious emotional disturbance (SED).
Exit Criteria #7:
 
Demonstrated provision of service to adults (aged 18 and over).
 
Demonstrated provision of service to adults (aged 18 and over) with serious mental illness (SMI).
Exit Criteria #9: 
 
Demonstrated provision of supported housing for adults (aged 18 and over) with SMI, within 45 calendar days of referral. 
Exit Criteria #10:
 
Demonstrated provision of supported employment services for adults (aged 18 and over) with SMI within 120 calendar days of referral.
Exit Criteria #11:
 
Demonstrated provision of assertive community treatment (ACT) services for adults (aged 18 and over) with SMI within 45 calendar days of referral.
Exit Criteria #12:
 
Demonstrated provision of newer generation anti-psychotic medications to adults with schizophrenia.
 
Demonstrated provision of services to adults (aged 18 and over) who are chronically homeless and seriously mentally ill.
Exit Criteria #14: 
 
Demonstrated provision of services to children and youth with SED in natural settings.
Exit Criteria #15:
 
Demonstrated support for children/youth with SED to live in their own home or surrogate homes.
 
Demonstrated provision of services to children/youth who are homeless.
Exit Criteria #17:
 
Demonstrated continuity of care upon discharge from inpatient facilities for adults, children and youth.
 
Demonstrated increase in the proportion of total resources directed to community services.
 
Demonstrated maximization of use of Medicaid funding to support community-based services
