Bulletin ID: No. 69 - MUI/UI Form Adds a Line In Section A, #7 (DMH Policy 480.1B Exhibit 4)

Effective immediately, providers are to use the newly revised MUI/UI Form. The “Name of the Provider Submitting the MUI/UI Report “ has been added to DMH Policy 480.1B Exhibit 4, Section A,  #7. It is defined in the MUI/UI Report Form Glossary ( DMH Policy 480.1B Exhibit 5).
